Data and purpose
The form requests name, phone or email, centre, contact preference and an administrative reason. Data is used only to reply, manage an appointment or handle the requested administration.
We do not request clinical data in the form. Do not enter symptoms, diagnoses, reports, images or identity documents.
Legal basis and retention
The legal basis is consent and, where applicable, pre-contractual steps requested by the person. Data is retained as needed to reply and manage the relationship and then for applicable statutory liability periods.
Recipients and providers
Data is not shared for marketing. Hosting, email or technical maintenance providers may process data under data-protection obligations, together with disclosures required by law.
Rights
You may request access, rectification, erasure, objection, restriction or portability by writing to askabide@askabide.com and identifying the request. You may also complain to the Spanish Data Protection Agency.
Health data and insecure channels
Do not send sensitive clinical information by WhatsApp or ordinary email. Clinical records are handled through the applicable internal clinical channels.
